Dijagnostikovanje celijakije

Jedna od najvažnijih grešaka je uklanjanje glutena pre testiranja. To znatno otežava dobijanje konačne dijagnoze.

Zašto morate nastaviti da jedete gluten

Celijakija se dijagnostikuje otkrivanjem imunološkog odgovora na gluten i oštećenja creva koje uzrokuje. Uklonite gluten i oboje nestaje: antitela opadaju ka normalnim vrednostima tokom nedelja do meseci, a resice počinju da se oporavljaju. Testirajte se u tom trenutku i rezultat je neinterpretabilan.

Rešenje je izazov sa glutenom — namerno ponovo uvođenje glutena tokom nekoliko nedelja pre testiranja — što je dovoljno neprijatno da mnogi ljudi odustanu i završe sa doživotnom restriktivnom dijetom i bez dijagnoze. To je važnije nego što zvuči: bez dijagnoze možda nećete dobiti praćenje gustine kostiju, specifičnu dijetetsku podršku za celijakiju, ili u nekoliko evropskih zemalja prava na lekove i subvencije koja dolaze s tim.

Smernice iz 2025. [2] i smernice Američkog koledža gastroenterologije slažu se oko ove opšte strukture, sa razlikama na marginama — posebno oko toga kada se biopsija može izostaviti.

Dijagnostički put

Potencijalna celijakija

Zbunjujuća kategorija: pozitivna serologija, normalna biopsija. Sistematski pregled i meta-analiza u časopisu Gut ispitivali su kliničke ishode u ovoj grupi, što je važno jer pitanje upravljanja — lečiti sada ili pratiti — nema očigledan odgovor. Mnogi, iako ne svi, napreduju ka atrofiji resica tokom vremena.

Ko treba da se testira

  • Hronična dijareja, nadutost ili neobjašnjiv gubitak težine.
  • Anemija usled nedostatka gvožđa bez očiglednog uzroka — veoma česta prezentacija, i jedna gde smernice gastroenterologije posebno ističu testiranje na celijakiju [7].
  • Prvi stepen rodbine sa celijakijom, gde je prevalencija otprilike deset puta veća od osnovne stope.
  • Dijabetes tip 1, autoimuna bolest štitne žlezde i nekoliko drugih autoimunih stanja koja se grupišu sa njom.
  • Neobjašnjeno povišene transaminaze, ponavljajuće čireve u ustima ili ranu osteoporozu.
  • Dermatitis herpetiformis, koji je zapravo celijakija kože i sam po sebi je dijagnostički.

Napomena šta nije na toj listi: nadutost sama, u odsustvu bilo čega drugog, je daleko češće sindrom iritabilnog creva — a smernica ACG o upravljanju IBS-om [4] je relevantniji dokument za većinu tih prezentacija. Celijakija i IBS se dovoljno preklapaju da razlika zaista zahteva testiranje umesto nagađanja.

Često postavljana pitanja
