Zašto je lek teško razviti

Celijakija ima neobičnu osobinu među autoimunim stanjima: okidač je poznat i može se ukloniti. To postavlja veoma visoke standarde — svaki lek mora biti sigurniji i praktičniji od jednostavnog nejedanja glutena, u populaciji koja nije akutno bolesna nakon lečenja. Takođe, to znači da realističan cilj nije "ponovo jesti hleb" već "biti zaštićen kada restoran pogreši".

Inhibicija transglutaminaze 2

Tissue transglutaminaza modifikuje peptide glutena na način koji ih čini mnogo imunogenijim — to je enzim u središtu mehanizma bolesti i cilj antitela koje se koristi za dijagnostikovanje. Njegovo blokiranje je stoga mehanistički elegantno. Randomizovano ispitivanje TG2 inhibitora izvestilo je o ublažavanju oštećenja sluzokože izazvanog glutenom tokom izazova [5], što je najhrabriji pojedinačni rezultat koji je oblast proizvela.

Enzimska degradacija

Glutenaze imaju za cilj da razgrade gluten u stomaku pre nego što stigne do tankog creva. Inženjerski problem je ozbiljnost: da bi se zaštitili od normalnog obroka pšenice, enzim mora da razgradi grame proteina u neprijateljskom kiselom okruženju u kratkom vremenskom okviru. Nekoliko kandidata je pokazalo aktivnost protiv malih količina bez demonstriranja zaštite od realističnog izlaganja. Rad na probiotskim preparatima sa aktivnošću razgradnje glutena in vivo ilustruje pristup.

Probiotici

Postoji sistematski pregled i meta-analiza randomizovanih kontrolisanih ispitivanja probiotika za celijakiju i ne podržava rutinsku upotrebu. Razlike u mikrobiomu kod celijakije su stvarne i reproduktivne, ali njihovo modifikovanje nije dovelo do klinične koristi.

Druge pravce

  • Indukcija imunološke tolerancije. Pristupi zasnovani na peptidima koji imaju za cilj da ponovo obučavaju imunološki odgovor, konceptualno slični imunoterapiji alergena.
  • Modulacija čvrstih spojeva. Smanjenje intestinalne propusnosti kako bi manje peptida glutena prešlo. Ispitivanja su bila mešovita.
  • Blokada citokina. Usmerena na refraktornu bolest, a ne na običnu celijakiju [7].
  • Vakcinacija. Dugo raspravljana, više puta odložena i nije blizu.

Šta očekivati

Verovatni sledeći korak je dodatak koji smanjuje posledice nenamernog izlaganja za ljude koji su već strogi. To bi bilo zaista dragoceno — nenamerno izlaganje je uobičajeno, kao što pokazuje istraživanje o kontaminaciji — bez da predstavlja slobodu koju izveštaji obično obećavaju.
