Pam mae meddyginiaeth yn anodd

Mae gan glefyd coeliac eiddo anarferol ymhlith amodau autoimmune: gwyddom a gellir tynnu'r trigger. Mae hynny'n gosod bar uchel iawn — rhaid i unrhyw feddyginiaeth fod yn ddiogelach ac yn fwy cyfleus na dim ond peidio â bwyta glwten, mewn poblogaeth nad yw'n sâl yn ddifrifol unwaith y bydd wedi'i thrin. Mae hefyd yn golygu mai'r targed realistig nid yw "bwyta bara eto" ond "bod wedi'i amddiffyn pan fydd y bwyty'n gwneud camgymeriad."

Inhibition transglutaminase 2

Mae transglutaminase meinwe yn newid peptidau glwten mewn ffordd sy'n eu gwneud yn llawer mwy immunogenic — ef yw'r ensym yn y canolbwynt o'r mecanwaith clefyd ac yn darged yr antibody a ddefnyddir i'w ddiagnosio. Mae ei rwystro felly'n foddhaol yn fecanegol. Adroddodd treial rhifedig o inhibitor TG2 leihau niwed mwcosaidd a achoswyd gan glwten yn ystod y sialens [5], sy'n'r unig ganlyniad mwyaf cynnil y mae'r maes wedi'i gynhyrchu.

Dadelfeniad ensym

Mae glutenases yn ceisio torri glwten i lawr yn y stumog cyn iddo gyrraedd y coluddyn bach. Mae'r broblem beirianyddol yn ddifrifol: i amddiffyn rhag pryd o fwyd gwenith arferol, mae'n rhaid i ensym ddadelfennu gramau o brotein mewn amgylchedd asidig gelyniaethus o fewn ffenestr fer. Probiotig

Mae adolygiad systematig a meta-analys o dreialon rheoledig rhifedig o probiotics ar gyfer clefyd coeliac yn bodoli ac ni chefnogir eu defnydd rheolaidd. Mae gwahaniaethau microbiome yn clefyd coeliac yn real ac yn ailadroddadwy, ond ni chafodd eu newid i fudd clinigol.

  • Indwyo goddefgarwch imiwn. Dulliau seiliedig ar beptid sy'n ceisio ailhyfforddi'r ymateb imiwn, yn gysyniadol tebyg i therapi imiwnedd allergen.
  • Modulation cyffyrddiadau tynn. Lleihau tryloywder coluddyn fel bod llai o beptidau glwten yn croesi. Mae treialon wedi bod yn gymysg.
  • Blocio cytokine. Aimed at clefyd gwrthod yn hytrach na'r clefyd coeliac arferol [7].
  • Brechlyn. Wedi'i drafod yn hir, wedi'i ohirio'n gyson, ac nid yw'n agos.
